Systemic inflammation may be associated with disease activity in multiple sclerosis (MS). The relationship between systemic inflammation and MS has not been previously established. The aim of this study was to measure plasma levels of interleukin-6 (IL-6) and C-reactive protein (CRP) in patients with relapsing-remitting MS (RRMS) over a period of one year, and to assess their correlation with clinical, immunological, and magnetic resonance imaging (MRI) measures of disease activity. Plasma IL-6 and CRP levels were measured in 99 RRMS patients at baseline, 12 months and 36 months (n = 22 at 12 months). Disease activity was assessed using the Expanded Disability Status Scale (EDSS) and gadolinium-enhanced MRI at baseline and after 36 months of follow-up. Longitudinal analyses demonstrated that plasma levels of IL-6 and CRP were significantly higher in patients with new and/or increased disability compared to patients with stable disability. Plasma IL-6 and CRP were strongly correlated with each other (r = 0.60, P 1.2 pg/mL had a ≥1.0 EDSS point increase during follow-up than those with lower IL-6 levels. Systemic inflammation was also associated with new or increased disability. IL-6 and CRP may be potentially important biomarkers of MS progression.
